All patients are required to print and sign the HIPAA compliance forms.
Male Patients 50 and older or any male with a family history
of prostate cancer please print 1 & 3.
All other patients please print 2 & 3.
Please print these forms and complete them. Bring these
completed forms with you for your first visit.
Here is a list of other items you will need for your first
visit:
1. Your insurance card (s)
2. A list of all your medications
3. All new patient forms you printed and completed
from this site
4. Any copay that may apply. We do not take credit
or debit cards, only cash or check. There is a $3.00 service fee if the copay is not paid at the time of service.
Please be sure to arrive 15 to 20 minutes before your first
visit to ensure that all paperwork has been completed.
